Enter Lot Number:    

1. Please enter the complete Lot Number in the box above. The Lot Number can be found on the product label.

2. Click the "Look Up" button.

3. A new button should appear below the "Look Up" button that says "Open" and the Lot Number that you entered.

4. Click the "Open" button.

5. A PDF will open in a new window which will allow you to save or print the document.

6. To download another Lot Number, go back to the "Look Up" feature and enter a new Lot Number. Repeat steps above.
